Dear Fred,

I am pleased to answer the inquiry regarding the labelling
of.Repoise (butaperasine), limiting its application to patients with
chronic mental disorders.

I have

studied butaperasine since

1964,

assessing the

clinical effects in chronic schizophrenic patients; the EEC and clinical
relations; the effects of'concentrate (syrup) Repcise in acute mentally
and in association with Dre. Itil and Holden at the Missouri
ill;
Institute of‘Psychiatry, have been acquainted with their data on the
effects on clinical laboratory tests and the EKG.
In these studies, and in reading the published data, we
to be an effective antipsychotic agent with a
low profile of'extrapyramidal effects. Dosages can be increased rapidly
and the therapeutic effect can often be observed in 3 to 4 days. In its
EEC profile, putaperasine exhibits a characteristic hypersynchrony of
alpha activity, enhanced amplitudes, decreased variability of'frequencies,
and in higher dosages, enhanced theta and delta activity with a reduction
in fast frequencies. This profile is similar to piperasine phenothiaaines,
such as fluphenasine.

In the clinical studies, its activity is similar to fluphenasine,
in acute and chronic cases. From this point of’view, the implied
limitation to chronic cases appears arbitrary:
fhe difficulties in differential diagnosis is great, and
is
it often impossible to decide whether the patient is acutely ill,
a relapse in a chronic illness, or an acute exacerbation in the
course cf'a chronic illness.

or a part of an anxiety syndrome; similarly, the sypptom of psychomotor
agitation often responds whether in schizophrenia, depressive psychosis,
or senile agitation.
In

my

experience the decision to use a therapeutic agent
on a clinical diagnosis, but on a symptom profile,

is usually based not

it

and some elements of history. From this point of view,
seems arbitrary
and an unnecessary implied limitation on the decisions of the physician
to continue the labelling restriction for Repoise for chronic cases.
From another point of view, the limitation restricts the
teaching of the use of Repoise. In teaching residents the principles
of psychopharmacology, it is a special burden for the teacher to attempt
to describe the use of butaperazine and note its limitations. It is
much easier to recomend the use of fluphenazine, for which no such
limitation is implied (and incidentally, for which a variety of form-

ulations exist).
