Dear

Miss Harriet Bernstein, age 39, was seen at Gracie Square
Hospital on December 9 and in our EEG laboratory on December l9.
At Gracie Square she presented a mild organic mental syndrome,
moderate to severe depressive state and evidence of barbiturate
intake. She was mildly ataxic, labile and her Speech was slurred.

when seen on December l9, she was no longer ataxic nor did
she have nystagmus. The mental syndrome had been relieved and
she appeared anxious. tense and depressed. She had a pressure
of speech.
The electroencephalogram (#039) was symmetric without evidence
of a focus. The rhythms were highly irregular with mixed theta,
alpha and a moderate degree of high voltage beta activity. The
beta activity did not appear in-spindles or runs. The frequency
of theta activity was 5-6 cycles per second, with amplitudes between
40 and 80 microvolts. The record was filled with eye blinks
artefacts despite manual control.

Recommendation: In view of the history, we cannot exclude
the possibility that the dysrhthmia is related to previous convulsive
treatment, although the probability is Very small. With this in
.mind I would suggest that she be continued on Oilantin treatment
and that the depressive state he treated with imipramine. If imipramine
is given rapidly in the hospital setting, we will know if dysrhythmia
is enhanced.,if the EEG is repeated when she is stabilized at your

average imipramine dose.
